Our self pay rates allow our patients to receive the same treatment from their primary care physician (PCP), with an office visit charge of $150 to establish our provider as a PCP and $90 follow up appointments. Vaccines and therapeutic injections may be subject to additional fees. Acute/Urgent Care visits for minor illnesses and injuries, we offer a $195 package that will include a medical examination, labs and testing along with your prescription medications that will be dispensed by our affiliated pharmacy: Oak Lawn Pharmacy. Full payment is due at the time of services, including any additional charges. Not valid for Workers Compensation.
